Gather Health is transforming primary care for older adults through an innovative Social Primary Care model. As a Primary Care Physician , you'll provide comprehensive, relationship-based care to clinically and socially complex older adults while helping patients achieve their health goals through longitudinal care delivered in our clinics and via telehealth.

Working alongside an integrated care team, you'll focus on whole-person care that improves both individual and community wellness.

Required Qualifications

• Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatrics
\n
• Active Massachusetts medical license, or ability to obtain one prior to start
\n
• Clinical proficiency in evidence-based primary care
\n
• Experience utilizing EHRs and healthcare technology
\n
• Strong communication and interpersonal skills
\n
• High ethical standards with the ability to manage sensitive situations professionally
\n
• Passion for mission-driven, patient-centered care
\n
• Ability to thrive in a collaborative, non-hierarchical environment
\n
• Ability to effectively balance competing priorities
\n

Preferred Qualifications

• Experience caring for older adults in underserved communities with complex social challenges
