Home Health LPN
Job Summary:
We are seeking a compassionate and skilled Home Health LPN to join our team at Orion Home Health in Humboldt Park, IL and the surrounding areas. As a Home Health LPN, you will play a vital role in providing high-quality, patient-centered care to individuals in the comfort of their own homes. Responsibilities:
Provide skilled nursing care to patients in their homes, following established care plans and protocols
Conduct patient assessments, develop care plans, and implement interventions to improve patient outcomes
Work collaboratively with physicians, therapists, and other healthcare professionals to ensure comprehensive care
Educate patients and their families on self-care techniques, disease management, and prevention strategies
Document patient data accurately and timely, maintaining confidentiality and adhering to HIPAA standards
Participate in case conferences, team meetings, and other activities to promote effective communication and coordination of care
Requirements:
Current LPN license in the state of Illinois
Minimum 1 years of experience in home health or a related field (new graduates welcomed to apply!)
Ability to work independently and as part of a team
Strong communication and interpersonal skills
Reliable transportation and willingness to travel to patient homes throughout the Chicago area
