Hospice Full-time RN or LVN

Location: College Station and surrounding area

What You’ll Do:

• Manage a caseload of hospice patients in the College Station area.
• Provide skilled nursing care in patients’ homes and facilities.
• Complete routine visits, admissions(RN), and recertifications(RN).
• Develop and update individualized plans of care(RN).
• Collaborate with physicians and the interdisciplinary hospice team.
• Provide education and emotional support to patients and families.
• Maintain accurate documentation using a company-provided tablet.
  

What We’re Looking For:

• Active Texas nursing license (RN or LVN).
• Strong assessment and critical thinking skills.
• Excellent communication and teamwork.
• Ability to work independently in the field.
• Reliable transportation and willingness to travel locally.
• Most importantly, we’re looking for nurses who lead with compassion, patience, and empathy.
